Using these Procedures (Reablement)

In December 2023, further information about the reablement service was added into Section 1, About the Service.
There are 5 area reablement services that operate across the county. They are all registered with the Care Quality Commission.

Eligibility
Eligibility is on a case-by-case basis but people would usually:
- Have reablement goals;
- Have given consent to have a reablement service (or this has been deemed in their best interests under the Mental Capacity Act 2005);
- Be over the age of 18; and
- Have the potential to gain skills.
The Reablement Service
The Reablement service is available free of charge for a few days up to a maximum of 6 weeks (depending on individual need), to anyone who would benefit from a skills gain programme to increase their level of independence.
Reablement assistants work with people to achieve specific goals and then put together a support plan to help maximise potential to achieve these goals. Sometimes this will also involve support from other professionals such as Occupational Therapists.
If longer term support is needed, reablement assistants will work with staff in local social care teams to help meet longer term support needs.
These procedures should be used by the reablement service when:
- Receiving a contact or referral
- Establishing reablement needs (where an assessment of needs has not been completed)
- Reablement planning
- Reviewing reablement
The element of the service regulated by the Care Quality Commission should refer to the Universal Procedures as required, and to guidance in the Local Resources of outside of this site.
