Connect to Community (C2C) service. All new referrals go via the Information and Advice Service and will be progress to Connect to Community Teams. C2C will be the front door for AWHP, responding to referrals for cases not already open to the service including Safeguarding concerns. Connect to Community will focus on a strengths based, preventative approach to manage demand and provide a robust response to residents.

There are 4 C2C Teams:

  • West 1 (covering Guildford, Waverley and Surrey Heath);
  • West 2 (covering Woking, Spelthorne and Runnymede);
  • East 1 (covering Reigate and Banstead and Tandridge);
  • East 2 (covering Epsom, Mole Valley and Elmbridge).

Following Connect to Community Teams intervention, if there is the appearance of eligible care and support needs that cannot be met within Connect to Community, cases will be passed through to the Area Teams for ongoing support and assessment. 

The Area Teams compromise of social workers and social care assistants. Frontline staff working in Area Teams assess and support the needs of adults with care and support needs and carers with support needs, unless the adult's primary support needs are best met by specialist mental health, sensory or learning disability teams.

There are 4 Area Teams:

  • West 2 (covering Woking, Spelthorne and Runnymede);
  • East 1 (covering Reigate and Banstead and Tandridge);
  • East 2 (covering Epsom, Mole Valley and Elmbridge).
