Using these Procedures (Connect to Community Teams and Area Teams (Occupational Therapy))
Occupational Therapists will work within the Connect to Community (C2C) service. C2C will be the front door for Adults, Wellbeing and Health Partnerships, responding to referrals for cases that are not already open to the service, including safeguarding concerns. Connect to Community will focus on a strengths based, preventative approach to manage demand and provide a robust response to residents.
Occupational Therapists within C2C will also support those with long-term ongoing needs.
There are 4 C2C Teams:
- West 1 (covering Guildford, Waverley and Surrey Heath);
- West 2 (covering Woking, Spelthorne and Runnymede);
- East 1 (covering Reigate and Banstead and Tandridge);
- East 2 (covering Epsom, Mole Valley and Elmbridge).
Occupational Therapists will also be based within the:
- Learning Disability and Autism Teams;
- Transition Service;
- Enabling Independence Teams.
These teams will continue to support new referrals into the service as well as manage existing case work.
