In December 2023, this team name was changed to 'Communities and Prevention Coordinator Service' and information in Section 1, The Communities and Prevention Team was updated.
The Communities and Prevention Coordinators (CPCs) are part of the wider Communities and Prevention Team. CPCs work within the principles of the Care Act 2014 and aim to prevent, delay, and reduce social care needs from arising.
Their vision is to bring communities together, connect people to support and work in partnership with others. The team want residents to flourish, receive the right support and information, empower resident participation and work across all communities.
CPCs respond to LAS referrals (using the bespoke referral document) from adult social care teams (localities, hospital teams, LD&A and Mental health Teams) supporting practitioners to find free or low-cost community services. CPCs don’t have direct client contact for their role within the ASC teams, however, will work with members of communities when needed.
CPCs find out about and share information about local community services and maintain a database of support available in local communities. They also identify gaps in need and work closely with communities to promote projects that bridge those gaps.
CPC’s also work closely with district and borough partners as well as with colleagues in the Customer and Communities directorate, especially the Local Area Coordinators and Communities Link Officers.
