About the Service (The Transition Team)

The Transition Team supports young people, who are likely to have care and support needs following their 18th birthday through the transition from Children’s Services to adult social care.

As well as having likely care and support needs from the age of 18, the young person must also have an Education, Health Care Plan (EHCP), and a formal diagnosed learning, physical or sensory disability and/or autism. Without this the referral will not be accepted.

Note: The Transition Team in Surrey can only support people that are resident in Surrey. 

Please call the Team on 01276 800 270 prior to making a referral if:

  • The young person, or their parents, currently live outside of Surrey;
  • Any aspect of the young person's care is, or has been, provided by another Local Authority.

Referrals can be made from the time a young person is 14.

Referrals must be made by professionals.

The following information must be provided when making a referral:

  • Copy of the latest EHCP - dated no more than 18 months ago.
  • Formal evidence of diagnosis - this is mandatory - speak to GP to gain documentation.
  • CHC referral information - if submitting from Children with Disabilities or Looked After Children teams.
  • Family contact details.
  • Details of education background, achievements to date.
  • Contact details of all involved professionals.
