About the Service (Learning Disability and Autism Service)

The Learning Disability and Autism Service is a countywide service, that is comprised of the East, West and Project Teams. These teams provide statutory support to working-age adults who present with care and support needs pertaining to learning disability and/or autism.

To be eligible for a service from one of the teams, a person must have a relevant diagnosis of learning disability and/or autism and meet the eligibility criteria as defined by the Care Act 2014.

The person needs a balance of primary needs related to their diagnosis of either a learning disability or autism. Where cases have a greater balance of other primary needs (such as physical needs, or mental health needs) then the case would not be eligible for the service.

Cases that meet the criteria as defined above, and which are to be transferred to the Countywide Learning Disability and Autism Service should be transferred only once this has been agreed by a Manager (ATM, TM, SM) within the service.

Case transfers should be done in a planned way involving the person and their family/carers. No cases will be accepted for transfer unless there is an up to date assessment, and a review or reassessment has taken place within the last 8 months.
